Description
Assists in the selection of safe and adequate space, setting, equipment and supplies for activities programs that are person centered and culturally diverse. Transports residents per care plan to activity, events meals and visitations. Is available for scheduled weekend and evening activities at a minimum of two times per month. Carries out group/individual activities programs independently; including set up and clean up of activity. Possesses knowledge of a variety of leisure time activities and is able to teach these to the residents. Coordinates planned activities with the patient/resident schedule of other services. Engages residents in therapeutic task groups specialized for resident with dementia related diseases. Assists with cleaning and preparing the dining room for meals (Care Unit only). Provides redirection to residents who are attempting to self-ambulate. Alert appropriate staff as necessary. Assists in reviewing the patient/resident’s activities participation and revises the plan as necessary with the Activities Director or Assistant. Assists with any necessary transportation of patient/residents via Geri/wheelchair to and from programs. Provides guidance/direction of ambulatory patients/residents when necessary. Assist with data entry related to daily individual attendance records for patients/residents. Supervises and orients volunteers, newly hired staff and pastoral care personnel. Assists in the development of monthly activities schedule based upon individual and group needs, interests and capabilities. Assists with posting the current monthly activities calendar where it is accessible to patients, residents and staff and can be easily read.
Possess conflict management skills to avoid disruptive behavior between patients/residents. Intervenes and redirects as needed. Participates in facility CQI meetings as directed by the Activities Director. Maintains resident rights. Maintains HIPAA privacy, security and confidentiality standards. Follows all established policies and procedures. Follows universal Precautions and Infection Control techniques. Maintains Corporate Compliance standards. Contributes to facility Continuous Quality Improvement initiatives. Follows safety procedures by keeping areas free of hazardous conditions. Economical and careful with supplies and equipment. Attends all required in-services. Comes to work as scheduled and consistently demonstrates dependability and punctuality. Comes to work in neat, clean attire and consistently presents an appropriate professional appearance. Prepares any other necessary duties as assigned by Supervisor.
