Description
The RN Case Manager is responsible for providing evaluation and nursing services on an intermittent basis in the patient's place of residence. Functions independently and/or in conjunction with an interdisciplinary team based on the patient's identified needs. Practice is within the scope defined by the Washington State Nurse Discipline Practice Act, based on the nursing process model and concept of community health nursing therapy.

Required qualifications:
Graduate of an accredited school of nursing
Upon hire: Washington Registered Nurse License
Within 30 days of hire: National Provider BLS - American Heart Association
Within 30 days of hire: National Provider Identifier (NPI) number

Preferred qualifications:
Bachelor's Degree
2 years full time experience practicing as a registered nurse in a Long Term Care, Medical/Surgical Unit, and/or Skilled Nursing Facility.
1 year recent, hands-on technical clinical experience (occurring within the last three years) in a Medicare-certified Homecare or Hospice agency/facility doing intermittent visits and case management.
Recent hands-on experience in wound care, venipuncture, ostomy, catheters, and feeding tubes.
Rehab nursing experience.
Experience working with an EMR system.
Experience with office software programs including Outlook, Word, and/or Excel.
