Job Summary
The mission of University Neighborhood Partners ( UNP ) is to bring together university and west side people and resources in reciprocal learning, action, and benefit — a community coming together. UNP convenes faculty, students, west side residents, community organizations, and agencies to work together to address resident-identified community priorities. By addressing social determinants of health and systemic barriers to educational success, these collaborative partnerships foster increased access to higher education for west side residents, a university enriched by its involvement in the broader community, and an enhanced quality of life for all involved.
Under the supervision of a Licensed Clinical Social Worker (L.C.S.W.) this position is responsible for integrating mental health and capacity building supports in efforts to increase overall community health, wellness and access to culturally responsive care for west side residents.
The Clinical Health Partnership Manager will develop, and support community-campus partnerships focused on culturally responsive health and wellness priorities. These partnerships address community identified priorities while simultaneously building the capacity of partners to find long-lasting, sustainable solutions that lead to equitable system change.
This position supports clinical licensure pathways for early career CSW’s through clinical supervision and direct clinical opportunities with health partners including Hartland Bridge Clinic, Hartland Free Mental Health Clinic and Population Health Center.

Responsibilities
Essential Functions
Identifies community priorities involving health and wellness in west side neighborhoods. Develops and implements culturally responsive interventions addressing community priorities. Develops and supports community-campus partnerships, including interdisciplinary partners at the University of Utah Population Health Center and Hartland Partnership Center. Evaluates individuals and families through interviews and psychosocial assessments to establish and develop treatment plans and goals with the licensed treatment team. Collaborates effectively with other community-based organizations and related agencies focused on community health and well-being. Identifies and provides support in the form of information and referrals for community resources to individuals and families such as shelter, transportation, future treatment plans and financial assistance. Develops and provides stress management/wellness treatment plans for outpatient populations. May perform some Case Management duties.

Ability to perform the essential functions of the job outlined above. Support as appropriate to the population served. Demonstrated human relations and effective verbal and written communication skills. Bilingual/multilingual skills: English as well as other languages spoken in west side neighborhoods (Swahili, Spanish, Somali, Karen, Tongan, Samoan, etc.). Experience, engagement in west side neighborhoods. Ability to work in a multidisciplinary team, problem solve, and resolve conflicts for individuals, couples, families and groups. Ability to identify and navigate community resources that provide support for individuals and families. Ability to make recommendations based upon experience regarding policy and delivery of service.Demonstrated knowledge and skills necessary to effectively facilitate community conversations.
Minimum Qualifications
Required
Current RQI Healthcare Provider eCredential through the University of Utah Health RQI system. The eCredential is to be obtained within 30 days of hire.
One of the following
Current Licensed Clinical Social Worker (L.C.S.W.) certificate for clinical practice in the State of Utah.
Current license to practice as a Certified Social Worker in the State of Utah.
This position is patient-sensitive and must fulfill all associated requirements.
